This document is a diagnostic questionnaire for Temporomandibular Disorders (TMD). TMD refers to conditions that affect the joint connecting the jawbone to the skull and the muscles used for chewing. <br /><br />The questionnaire begins by asking the patient about any pain experienced in the jaw, temple, ear, or in front of the ear. The patient is also asked to indicate the duration and frequency of the pain. Next, the questionnaire asks if certain activities like chewing tough food, opening the mouth, jaw habits, or other jaw activities have affected the pain.<br /><br />The questionnaire then moves on to ask about headaches in the temple areas of the head, including their duration and any changes in the headaches due to certain activities.<br /><br />The next section of the questionnaire asks about any jaw joint noises experienced while moving or using the jaw. The patient can indicate if jaw joint noise was present and specify if it was on the right (R), left (L), or both sides.<br /><br />The questionnaire also asks about locked or limited jaw movement. The patient is asked if their jaw has ever locked or caught, limiting their ability to open it all the way. If it has, they are asked about the severity of the lock or catch and any recent occurrences of jaw locking.<br /><br />Lastly, the questionnaire asks about open locking of the jaw, which refers to the jaw getting stuck in an open position. The patient is asked if they have experienced this and if they needed to do anything to close their jaw from this wide-open position.<br /><br />This questionnaire can help healthcare professionals in diagnosing and assessing Temporomandibular Disorders in patients.
