Overview
This lecture offers a comparative analysis of oral appliance therapy (OAT) using mandibular advancement devices (MADs) and hypoglossal nerve stimulation (HGNS/Inspire) in the management of obstructive sleep apnea (OSA). We will examine mechanisms of action, anatomical considerations, and clinical outcomes, including responder and non-responder profiles, based on current evidence. The session will also explore the role of combination therapy (OAT + HGNS) and present clinical cases that illustrate integrated, patient-centered treatment approaches.
Objectives
At the conclusion of this lecture, participants will be able to:
Identify anatomical factors and predictors of treatment success, as well as limitations, for both oral appliance therapy (OAT) and hypoglossal nerve stimulation (HGNS).
Evaluate clinical outcomes and responder criteria using evidence-based guidelines.
Review the importance of patient selection for achieving successful outcomes in combination therapy and analyzing clinical cases that demonstrate enhanced efficacy when OAT and HGNS are used together.

The target audience for this lecture is dental professionals, including general dentists, specialists, and clinicians interested in the management of sleep-related breathing disorders and orofacial pain.
Speaker Bio
Harmeet K. Chiang, DDS, MS, FAGD, is Associate Professor, Vice Chair, and Program Director of the Advanced Education in General Dentistry (AEGD) program at Virginia Commonwealth University (VCU) School of Dentistry. She is a Diplomate and the current President-Elect of the American Board of Dental Sleep Medicine. She founded the dental sleep practice at VCU and is nationally recognized for her leadership in dental sleep medicine. Dr. Chiang received the Michael O’Neill McMunn Distinguished Service Award and the AADSM Excellence in Clinical Research Award for her contributions to clinical research. She has published extensively in peer-reviewed journals in the field. At VCU, she redesigned the occlusion curriculum and has been honored with the ADEA Clinical Faculty Award and Dean’s Awards for Humanism and Didactic Teaching. She mentors students and leads initiatives advancing inclusive education and oral health equity, underscoring her lifelong commitment to humanistic care and transformative dental education.
